A test case is a specific, documented scenario to verify (inputs, steps, expected result); a test plan is the higher-level document defining what will be tested, how, and by whom for a given release or feature.
Think of it like
A test plan is a restaurant's full health-inspection checklist; a test case is one specific line item on it — 'check that the walk-in fridge holds 40°F.'

Has written a test plan or case someone else actually executed without them in the room.
